Not the authoritative answer, but I've always interpreted "Is the power listed as affecting all creatures within a particular area, and a meaningful chunk of the Mob is in that area? Roll it straight." (first bullet point in the shaded box on p.153) to apply to mob-on-mob damage, since mob attacks "affect all creatures within a particular area".

See, mobs doing straight damage to other mobs means PC's mobs just murder other mobs because they go first. Add Blood Like Water and you can chum through every other mob in the area if you have one or two Large mobs of your own.

Case specific, I think. If armies are clashing, I would personally say no, as while there may be brawls going on, forces have reserves that are not fighting, which are likely significant. However, a mob of... pixie soldiers descending from the sky to engage the whole of the enemy force? I believe both sides doing straight damage there would be fine, as nothing is held back.
